Once you’re sure about your decision, you’ll schedule your appointment directly with Dr. Amy, so please have the following information available for your call.
Preferred Appointment Day, Date, & Time: We’ll do our best to make it work.
Your Contact Information: Name, phone number, and email address.
Home Address + Any Special Instructions: Please let us know of any potential map-related problems, directions for where to park, or other important information.
Pet Information – Name, age, breed, weight.

Once your appointment is scheduled with Dr. Amy, you’ll be asked to fill out a consent form on this website.
Please fill out this form completely.
You’ll be asked to provide the following additional information:
Referral: How did you find Dr. Amy? Online, through a friend, your vet, etc.
Sex of Pet: M/F
Clinic Name: For ashes delivery.
Presenting Condition: (If known)
Then, you’ll be asked to check boxes to make the following choices:
Euthanasia (Humanely terminate life)
Retain Pet For Burial: You must discuss this with Dr. Amy
Communal Cremation: Cremation with other pets. Ashes are distributed in a beautiful area of Colorado
Private Cremation: Allows you to retain your pet's ashes in your choice of one of the urns or a satin pouch
Pawprint Keepsake & Color Choice: White or terracotta
Ink Paw or Nose Print
And finally…Note any other Special Requests

Once your consent form is submitted, you’ll receive an email outlining your next steps before your scheduled appointment.
